Only eligible
patients will be considered for referral through VPN, meaning 200% or
less than Federal Poverty Guidelines and no access to insurance
(Medicare, Medicaid and VA Benefits).

In order to qualify economically,
you must be able to prove that as a household that you make 200%
or less of federal poverty guidelines. This means:
